Appointments

We believe all persons are personally responsible for the appointments they make, including the doctor. If we can't keep or need to change an appointment time, we will give you reasonable notice. We expect the same from our patients. The policy below is strictly adhered to because, since we don't double book patients, we are left with no patient to see during the time we reserved for you. Appointments must be canceled no later than midnight the day before to avoid "no show".  There are special exceptions for COVID19 symptoms, and we are waiving no show fees for Covid-19 positive patients who have missed appointments until further notice.

No Show Policy

 + Existing Patient: If he/she misses an appointment AND doesn't give one day's notice, we call and send a letter to them reminding them of our policy. If this occurs again the patient must pay $50.00 to reschedule.

 + New Patient:
If he/she misses an initial appointment AND doesn't give a day's notice, there will be a $100.00 charge to reschedule.

Orthotics

We will gladly bill your insurance for orthotics but must require a credit card on file when ordering to cover our lab fees in the case your insurance does not cover them or coverage terminates prior to the dispense date. 

Payment Policy

 + We will gladly bill any insurances you may have.
 + You are responsible for any deductibles and co-pays at the time of service.
 + Past due accounts will be assessed an $8 re-billing fee for second statement.
 + Accounts over 60 days old will be sent to collections and an additional 33% will be assessed to the account balance.

New Patient Form

If you are a new patient, we will ask you to pick up or print our New Patient History Worksheet before you come in for your first appointment.   This will expedite your visit.  Thanks!
